Hospitals and MRSA
Hospitals take the central role in the fight against MRSA in a region. They carry the main responsibility in the engagement against MRSA in their region. Studies show that more than half of all MRSA in German hospitals are already detected at the point of patients admission. For this reason, the real implementation of the recommendations regarding the handling of MRSA in hospitals is urgent and necessary. This will only be successful if all hospitals of an epidemiologically linked region implement these recommendations. Isolated solutions will have no success and the hospitals which try to implement the recommendations will suffer as a result of those which do not accomplish these measures adequately. The basis of all efforts in the fight against MRSA in the hospital is:
- Prevention-Implementation of the infection control measures in accordance with guidelines- Isolation of all possible MRSA carriers and patients infected with MRSA- Controlled antibiotic prescription,
- Surveillance- the early laboratory based identification of carriers by screening of at-risk patients at admission-spa typing of the MRSA, in order to recognise transmission chains and to get an overview of the dynamics of the spreading and persistence of MRSA in the patient population,
- Therapy and decolonisation of the infected and/or colonised persons during and also after their stay in hospital
- Informing the public as well as the intensive training of personnel in the health care sector
- Regional and sector-crossing cooperation
The successful cooperation of the hospitals in the EUREGIO MRSA- net quality group depends on the accomplishment of the MRSA- net quality targets for the hospitals.
Quality targets for hospitals and clinics in the EUREGIO
- Participation at the EUREGIO MRSA net quality group meetings
- Collection of epidemiological data
- Training at the MRSA table with public health authorities
- Prevalence screening and screening at admission (consent, accomplishment, analysis)
- Specification of the number and kind of the in house-specific risk groups (RG) and subsequent screening
- spa typing of selected MRSA (e.g. outbreak strains)
- Implementation of the guidelines (hygiene measures, isolation, decolonisation)
- Implementation of surveillance of nosocomial infections and multiresistant organisms (using EPI MRSA software)
- Delivery of information regarding MRSA at discharge from the hospital
- MRSA Screening of risk patients- Number of swabs (screening control) - Number of S. aureus (quality control of swab/laboratory)
